The NHS will open so called “care traffic control” centres to aid hospital discharge across the whole of England as part of a package of measures to prepare services for winter.
The centres, which already operate in around a quarter of local areas, host NHS, social care, housing, and voluntary services staff in one place with the aim of locating and coordinating the best and quickest discharge options for patients, whether this is at home or in social care or community care.
